For lymphedema therapists and fitters
A custom flat-knit garment is only as good as its measurements. Small errors can mean a garment that rolls, cuts in or doesn't control swelling. These tips help get it right the first time.
Timing
Measure at the end of the reduction phase, once limb volume has plateaued.
Measure right after bandages are removed, before swelling returns.
Measure early in the day if the patient isn't bandaging.
Technique
Use the manufacturer's measuring form for the specific product. Measurement points differ by brand.
Mark measurement points on the skin before measuring, so circumferences and lengths line up.
Measure with the patient in the right position: usually standing for legs and arm relaxed at the side or slightly away from the body for arms, following the form.
Pull the tape snug but not tight. It should rest flat on the skin without indenting it, unless the form calls for a reduction.
Take lengths along the limb, not straight up and down.
Measure twice. Repeat any measurement that seems off.
Details that matter
Note skin folds, lobules and deep creases, especially at the ankle and behind the knee. These may need special shaping or a different style.
Record fibrosis or tender areas where pressure should be adjusted.
Include the hand or foot if a glove, gauntlet or toe cap is needed.
Describe the patient's ability to put garments on, such as limited grip or trouble bending, so we can suggest zippers or other options.
Specify compression level, style, top band and closures on the order.
Common mistakes
Measuring a limb that is still reducing
Measuring late in the day, when the limb is swollen
Using a generic form instead of the brand's form
Skipping measurement points or mixing up left and right
Pulling the tape too tight
