Every referralTherapist referral paperwork3 forms
Send referrals securely online, or fax with the cover sheet on top. Page 2 lists what the progress notes must include: the manufacturer, model, quantity, size and color of each item. If a therapist sends the order, Medicare (CMS) also needs the prescribing provider’s progress note documenting the lymphedema diagnosis.
