Referring a Patient: How to Send an Order and What to Include

For therapists, physicians and clinic staff

Quick answers to the two questions we hear most from referring offices: how to send a patient to us, and what paperwork we need to get the order approved.

How do I refer a patient?

Use whichever is easiest for your office:

We contact your patient within two business days, verify their benefit, handle the prior authorization, and keep you updated. More for providers here.

What documentation do you need from me?

  • A signed prescription

  • A progress note from the past six months listing each item to order: manufacturer, model, quantity, size and color

  • Current measurements, if you have them

  • The patient's demographic sheet with insurance

  • For pumps: evidence that conservative treatment has been tried

Therapist sending the order? Medicare (CMS) clarified in 2026 that the lymphedema diagnosis must be documented by the treating practitioner (MD, DO, NP, PA or CNS) in their own progress note. Therapist notes can't be used for the diagnosis, even if the provider co-signs them, so please send the provider's note too.

Every measurement form, sizing chart and order form we use is in Provider Forms, grouped by manufacturer, with a search box. If a form you need is not there, call (800) 883-1549 and we will send it to you the same day.
