For Providers & Therapists

The process for providers.

Send us the referral and we take it from there — documentation, benefit verification, prior authorization, fitting, billing and appeals. You will hear back from us when your patient is fitted.

Step 1

Three ways to send an order

Parachute Health is our preferred method — it removes the faxing, emailing and phone time, and gets product to your patient faster.

01

Parachute Health

Create a login, select from our catalog and pin Midwest Compression for future orders. Patients can opt in to communicate with us directly.

02

Fax

Send the completed packet to 800-886-4201. Everything you need is in the Provider Forms below.

03

Call or email us

Not sure what the patient needs, or what your plan requires? Call and we will work it out with you before anything is sent.

Step 2

What to send us

Compression pumps

Three documents

  1. E0651 or E0652 packet — completed, depending on the device.
  2. Demographic face sheet — including insurance, primary and secondary. Front and back of the cards if you have them.
  3. Recent signed progress note — within the past six months, listing the pump and garments to order: manufacturer, model, quantity, size and color.
Fax to 800-886-4201  or order digitally on Parachute Health.
Compression garments

Four documents

  1. Signed prescription — upper or lower extremity RX form.
  2. Demographic face sheet — including insurance information.
  3. Therapist or clinic progress note — within the past six months, listing each item to order: manufacturer, model, quantity, size and color.
  4. Prescribing provider’s progress note — documenting the lymphedema diagnosis.

Custom garments also need documentation supporting the medical necessity of a custom garment, plus measurements if you have them.

Therapist sending the order? Medicare (CMS) clarified in 2026 that the lymphedema diagnosis must be documented by the treating practitioner (MD, DO, NP, PA or CNS) in their own progress note. Therapist notes can’t be used for the diagnosis, even if the provider co-signs them, so please send the provider’s note too.

Fax to 800-886-4201  or upload with the progress note on Parachute.

Step 3

What happens next

Once the referral is in, your office is done. Here is what we do.

01

We contact your patient

Within two business days of receiving the referral.

02

We handle the paperwork

Documentation, benefit verification and prior authorization where it applies. No chart-note chasing for your office.

03

We fit the patient

A Certified Compression Therapist measures and fits, then sets up and educates on proper use of pump equipment.

04

We confirm back to you

You hear from us once your patient is fitted. We then handle billing, appeals, refills and replacement eligibility.

Before anything ships, we discuss financial responsibility with your patient so there are no surprises.

Step 4 · Stay informed

Automatic updates on every order you send us

Sign up once and we keep your office updated on every order you create with us, from referral to delivery: compression pumps, pump garments, ready-to-wear and custom garments. There is no need to sign up for each patient. We match your email to the orders you place, so use the same email you use in Parachute or on your referral forms.

The updates you will receive

  1. Referral received — confirmation that we have the order and will contact your patient within two business days.
  2. Missing paperwork — exactly what is still needed. Repeats weekly until everything is in.
  3. Benefits verified and ordered — benefits are verified and the pump or garment has been ordered.
  4. Shipped — with the same tracking number and link your patient receives.
  5. Delivered — your patient has their order.
  6. Patient has not signed their consent form — after several tries to reach your patient, we ask you to remind them at their next visit.
  7. Patient has not signed their delivery confirmation — the same reminder once the order is delivered.
  8. Patient payment pending — the order is on hold until your patient pays their portion. We sent them a secure payment link; we never share payment amounts or links with your office.

How updates reach you

Every update includes your patient's name, so we only send updates through secure channels: as a comment on the order for Parachute orders, and by secure Hushmail email or fax for all other orders.

When we ask for your help

If your patient has not responded, please ask them to check their text messages for the secure link we sent, email customersupport@midwestcompression.com, or call 800-883-1549 during business hours: Mon–Wed 9–3, Thu 9–5, Fri 9–1 ET.

Coverage — compression garments

What your patient's plan typically covers for garments

Most insurers have followed the Lymphedema Treatment Act, which created a benefit category for doctor-prescribed compression garments. The quantities below are the garment allowances most plans work to. We verify your patient's actual benefit in writing before anything is ordered.

3daytime garments per affected area, every 6 months
2nighttime garments per affected area, every 24 months
2business days — how quickly we contact your patient after a referral

Compression pumps are different. Pneumatic compression devices are not covered under the Lymphedema Treatment Act garment benefit — they are approved under separate device criteria, with their own documentation and prior authorization requirements. See the Pneumatic Compression Device Criteria above, or call us and we will tell you what the plan will want to see.

Have a patient who might benefit?

Send the referral, or call us first if you are not sure what the plan requires — garment or pump. We will tell you what is needed before your office spends time on it.

Midwest Compression is not billing Medicare or Medicaid at this time. All insurance plans vary, and most cover garments only with a specific diagnosis such as lymphedema. Cash pay is available for patients who do not fit insurance criteria. HSA and FSA cards and all major credit cards are accepted.
